Sports physicals have been set for Effingham Unit 40 students, only fifth through incoming 12th grades, Wednesday, May 20 from 4-7 p.m. at Sarah Bush Clinic on Maple Street.
Cost is $20 with all fees donated to the Effingham Sportsbackers. Appointments are required, please call 217-347-7776 or 217-347-7030 to schedule an appointment.
Immunizations will not be given. Cash or check only will be accepted. Please make checks payable to Sarah Bush Lincoln. Insurance will not be billed nor will insurance co-pays be collected.
A parent must sign the medical history form or be in attendance.
These physicals do not meet the kindergarten, sixth or ninth grade physical requirement.
Physicals are good for 395 days and are required in order to try-out, practice and play athletics at Unit 40.
