When: June 2–June 5

Where: Effingham High School Small Gym

Time: 8-10 a.m. Grades entering 3-5

Time: 10 a.m.-noon Grades entering 6-8

Time: 1-3 p.m. High School

What to bring: Gym shoes, practice clothes, and water bottle.

Fee: $40

* Mail the bottom portion of the form to Mindy McCollum-Boswell by May 26 at 602 South Pembroke Street Effingham, IL. 62401. If not received by the due date, your camper will not be guaranteed a T-shirt. Make checks payable to Effingham Volleyball.

I hereby authorize the staff of Effingham High Camp to act for me according to their best judgment in an emergency requiring medical attention and I hereby waive and release the camp staff and Unit 40 from any and all liability for any injury or illness incurred while at camp. I have no knowledge of any physical impairment that would be affected by the named camper’s participation in the camp.
