To address the needs of the communities it serves, Hospital Sisters Health System has launched HSHS Community Connect, a digital database of free or reduced cost community-based programs and services available in the communities served by HSHS.<\/div>\n

\u201cHSHS is committed to improving community health and addressing ever-changing needs and factors within the community landscape,\u201d said Kim Luz-Mobley, M.S., C.H.E.S., System Executive Director of Community Health. \u201cAcross our health system and the communities we serve, HSHS strives to fulfill our Mission to provide high-quality health care to all patients, regardless of their ability to pay.\u201d<\/div>\n

In 2024, HSHS hospitals conducted a\u00a0Community Health Needs Assessment<\/a><\/u> (CHNA) to identify the health status, needs and issues in the communities served. HSHS Community Connect is a tool that will support the goal to improve community outcomes by working to address social determinants of health, including access to medical care.<\/div>\n

\u201cAs a health care system,\u00a0we see members of our community every day who are experiencing food insecurity, homelessness, or in need of financial aid. With HSHS Community Connect, we can find local resources to connect our patients with the support they need,\u201d said Luz-Mobley.<\/div>\n
