Azlyn Rae Kollman, 25 days old, of Farina died unexpectedly at 8:23am Friday, July 15, 2016 at HSHS St. Anthony's Memorial Hospital in Effingham.

There will be a visitation for one hour on Thursday, July 21 beginning at 2pm at Gieseking Funeral Home in Altamont. A graveside service will follow at Union Cemetery, Altamont, with Rev. Rob Dillingham officiating. Memorials may be made to the donor's choice.

Azlyn was born at 2:58 p.m. on Monday, June 20, 2016 at Carle Foundation Hospital, Urbana to James R. Kollman and Keja L. Hovis of Farina. In her short time here on earth she was loved dearly and touched many hearts.

In addition to her parents she leaves behind; paternal great great grandparents, Eldon Campe of Altamont, and Peggy Mutschler of Kinmundy; maternal great great grandmothers, Loretta Holman of St. Elmo, Rose Williams of Toledo, Susan Brauer of Altamont; paternal great grandparents, James Kollman of Altamont and Debra Kollman of Jackson, Missouri, John and Teresa Durham of Farina and Lisa Durham of Portland, Tennessee; maternal great grandparents, Penny & Jay Buie of St. Elmo, Randy Hovis of Altamont and Dick & Ricki Morrison of Beecher City; paternal grandparents, Johnathan & Jamie Durham of Farina and James Campe of Altamont; maternal grandparents, Amanda Hovis and Michel Morrison of Beecher City; and many aunts, uncles and cousins.

She was preceded in death by her great great grandparents and cousins.
